A 12-year-old girl who died after an incident of self-harm was failed on multiple levels, an inquest jury at Sheffield Coroner's Court has found.
Mia Lucas, who died in January 2024, was found unresponsive at an the Becton Centre, an NHS children's psychiatric unit, after suffering an "acute psychotic episode" caused by a rare neurological disorder that had been left undiagnosed.
Chloe Hayes, Mia's mother, said she believes her daughter was "badly let down" by medics after a jury found that the failure to perform a lumbar puncture at the Queen's Medical Centre Nottingham, "possibly contributed" to her death.
